Determinar el diagnóstico y abordaje quirúrgico del frenillo labial aplicado a la ortodoncia

Resumen
Los frenillos bucales son bandas de tejido conectivo fibroso, muscular o de ambos, cubiertas de una membrana mucosa situadas en la línea media y de origen congénito. Existen tres frenillos: labial superior, labial inferior y lingual. Los frenillos pueden tener una estructura fibrosa, fibro-muscular o muscular. Sin embargo, estas estructuras pueden causar ciertas anormalidades o tipos de patologías, que son básicamente: Problemas ortodóncicoortopédicos, Problemas protésicos, Alteraciones fonéticas, Patología periodontal. El objetivo será, Determinar la técnica de abordajes quirúrgicos del frenillo labial para un tratamiento post-ortodoncico, mediante el método no experimental, descriptivo y bibliográfico. Por lo tanto cuando hay existencia de uno o varios signos clínicos se indica la cirugía. Deberá valorarse ante todo la edad del paciente, en especial cuando la indicación se deba a la existencia de un diastema interincisal. La cirugía eliminará el frenillo, pero sólo la ortodoncia cerrará el diastema. Algunos ortodoncistas prefieren hacer la exéresis quirúrgica o plastia del frenillo antes de cerrar el espacio, mientras que otros prefieren primero cerrar el diastema y hacer la cirugía después. Podemos marcar las siguientes pautas terapéuticas que correlacionan existencia de frenillo labial superior y diastema interincisivo: Frenillo labial superior en dentición temporal, Frenillo labial superior en dentición definitiva, Frenillo labial superior sin diastema interincisal. La remoción quirúrgica del frenillo representa solo la premisa, o un factor que favorezca el cierre del diastema, que es de competencia ortodontica. Los frenillos labiales a menudo se pasan por alto en el examen bucal sistemático porque rara vez son un problema. Sin embargo, estas estructuras anatomicas pueden causar ciertas anormalidades o tipos de patologías, que son básicamente: Problemas ortodóncico-ortopédicos, problemas protésicos, alteraciones fonéticas, problemas periodontales. Un frenillo demasiado hipertrófico puede provocar acumulación de alimentos y dificultar la limpieza de la zona. Palabras claves: Diastema interincisal, Patología periodontal, Frenillo labial.

Surgery will remove the bridle, but only orthodontics close the diastema. Some orthodontists prefer to surgical resection or plasty bridle before closing the space, while others prefer to first close the diastema and do surgery later. We can make the following treatment regimens that correlate existence of upper labial frenulum and diastema interincisivo: upper labial frenulum in primary teeth, upper labial frenulum in permanent teeth, upper labial frenulum without interincisal diastema.
