Resumen
El manejo de los dientes impactados, es probablemente el problema más común de la cirugía bucal en todo el mundo. Un diente impactado, es aquel que no erupciona en la arcada dentaria dentro del intervalo de tiempo esperado. Queda impactado, porque los dientes adyacentes, un hueso demasiado denso, un exceso de tejidos blandos o alteraciones genéticas impiden la erupción. El objetivo del presente caso clínico es determinar las ventajas que se obtienen al realizar la exodoncia del tercer molar inferior impactado, con una incisión angular, colgajo triangular y sutura individual. Se lo realizo, en las clínicas de cirugía bucal de la Faculta de Odontología de la Universidad de Guayaquil, mediante el análisis clínico en un paciente de 34 años de edad de sexo femenino. Los resultados demostraron que mediante la utilización de una incisión angular y colgajo triangular se obtiene mayor accesibilidad, irrigación y visibilidad para la extracción del molar impactado. Llegando a la conclusión, que el colgajo triangular es uno de los más recomendados si el cirujano necesita mayor acceso a las áreas más apicales del diente, que permite una mejor irrigación en el área de la intervención; además dicha incisión y colgajo favorece para una mejor cicatrización pos-operatoria del paciente. The management of impacted teeth, is probably the most common problem of oral surgery worldwide. An impacted tooth is one who erupts in the dental arch within the expected time interval. It is impacted because adjacent teeth, a bone too dense, excess soft tissue or genetic alterations prevent the eruption. The objective of this clinical case is to determine the advantages obtained when performing the extraction of impacted third lower molar, with an angular incision, single triangular flap and suture. I performed it in oral surgery clinics of the Faculty of Dentistry at the State University of Guayaquil, by clinical analysis in a patient of 34 years old female. The results showed that by using an angular triangular flap incision and greater accessibility, visibility irrigation and for the extraction of impacted molar is obtained. Concluding that the triangular flap is one of the most recommended if the surgeon needs better access to the most apical areas of the tooth, which allows better irrigation in the area of intervention; It said incision and flap also promotes better healing for postoperative patient. That should not be recommended as a general rule that impacted teeth are left as they are. If the tooth is left until problems arise at a later stage of life, it is likely to be complicated and dangerous surgery.
