Resumen
El retratamiento endodóntico es un procedimiento que consiste en realizar sobre
una pieza dentaria que ya tuvo un intento anterior de tratamiento definitivo que
termina en una condición que requiere intervención endodóntica adicional para
obtener un resultado exitoso. Esta situación puede darse por diferentes causas
que llevan al fracaso del tratamiento endodóntico realizado por primera intención.
Existen muchas situaciones por las que una endodoncia puede llegar a fracasar,
en este caso clínico el paciente acude a la Facultad de Odontología de la
Universidad de Guayaquil por un malestar con una restauración protésica en un
diente anterior, en el diagnóstico se pudo establecer que esa pieza presentaba un
poste intraconducto metálico sin obturación apical y adicional a esto también se
observó una lesión radiolúcida a nivel del periapice. El plan de tratamiento elegido
fue un retratamiento endodóntico con fines protésicos para así eliminar los
desechos bacterianos que estaban intraconducto, devolverle el soporte dentario
mediante la colocación de un poste prefabricado, la funcionalidad y la estética a la
pieza dentaria con la restauración de la misma con una corona metal cerámica.
Pero antes de llevar a término este tratamiento con éxito fue necesaria la
ejecución de unos procedimientos como la extracción de la corona y el poste
metálico mediante la utilización de fuerzas ultrasónicas; la realización de una
gingivectomía para obtener mayor remanente dentario y así lograr el objetivo
deseado. Este tratamiento obtuvo muy buen resultado tanto para los
requerimientos estéticos del paciente como en la parte fisiológica de la pieza
dentaria.
Endodontic retreatment is a procedure that is performed on a tooth that already had a previous attempt to definitive treatment ending in a condition that requires additional endodontic intervention for a successful outcome. This situation can occur for various reasons that lead to failure of endodontic treatment by primary intention. There are many situations that a root canal could fail in this clinical case the patient comes to the Faculty of Dentistry at the University of Guayaquil for an upset with a prosthetic restoration in anterior teeth, diagnosis could be established that intracanal pole piece had a metal without additional sealing apical and this level radiolucent periapical lesion was also observed. The treatment plan chosen was an endodontic retreatment prosthetic purposes in order to remove bacterial debris were Intracanal, restore the tooth support by placing a prefabricated post, functionality and aesthetics to the tooth with the restoration of the same with metal ceramic crown. But before this treatment to complete successfully implementing procedures such as removal of the crown and the metal post using ultrasonic forces it was necessary; performing a gingivectomy for more tooth remaining and thus achieve the desired objective. This treatment obtained very good results for both the aesthetic requirements of the patient and the physiological part of the tooth.
