Resumen
Existen diversas discusiones acerca de si los terceros molares provocan
maloclusion dentaria así como otras anomalías en la cavidad bucal como
la pericoronaritis, las caries, la reabsorción de las raices de los segundos
molares inferiores, la formación de quistes dentigenos, el desbalance
oclusal y alteraciones en la articulación temporomandibular.
Generalmente a los terceros molares se los observa con cambios en su
forma anatómica tanto en la corona, como en la raíz; además en mala
posición, por lo que no hacen oclusión y muchas veces tampoco hacen
erupción por lo que queda retenido en el hueso maxilar.
Algunos autores coinciden en que existe una relación estrecha entre la
posición que toman los molares y la perdida de espacio en los maxilares
lo que impide su enderezamiento factor que contribuye a la maloclusion
de las demás piezas dentarias.
Según diferentes estudios realizados se ha demostrados que el origen de
todos estos trastornos son genéticos ya que afectan mas a las personas
de raza blanca que a las personas de raza negra.
La posición de los terceros molares también determina el procedimiento
clínico a tomar por parte del profesional, ya que un diente parcial o
totalmente retenido dificulta la exodoncia, por lo que es necesario realizar
antes un estudio a fondo de cada caso en particular así como la adecuada
elección de la técnica a emplearse.
Para esclarecer tales problemas expuestos realizaremos una revisión
bibliográfica y presentaremos un caso de cirugía de tercer molar semi
impactado con el fin de adquirir conocimientos en estos aspectos del
sistema estogmatico.
There are several discussions about whether the third molars cause dental malocclusion and other abnormalities in the oral cavity as pericoronitis , caries , root resorption of the second molars , forming cysts dentigenos , the imbalance occlusal and temporomandibular joint disorders . Usually the third molars with the observed changes in anatomical shape both the crown and the root; well in bad position , so do not do occlusion and often do not rash which is retained in the jawbone . Some authors agree that there is a close relationship between position taking molars and loss of space in the maxillary preventing its straightening factor contributing to the malocclusion of the other teeth . According to different studies have achieved demonstrated that the origin of All these disorders are genetic and affecting more people whites than for blacks . The position of the third molars also determines the procedure clinician to use by professionals , since a partial tooth or retained completely hinders the extraction , so it is necessary before a thorough study of each particular case and the appropriate choice of technique to be used. To clarify these issues we will set a review literature and present a case of third molar surgery semi impacted in order to acquire knowledge in these aspects of estogmatico system.
