Alteraciones renales en pacientes VIH positivo estudio a realizar en el Hospital de Infectología Dr. José Daniel Rodríguez Maridueña durante el periodo de enero a diciembre del 2016

Resumen
De acuerdo a nuestra investigación confirmamos el universo total de pacientes VIH POSITIVO registrado por el Hospital, con un numero de 2.346 casos, de los cuales desglosamos el número determinado de pacientes que presentaron alteraciones renales, y tenemos 107 casos equivalente al 4% de del universo total registrados por el hospital. LAS ALTERACIONES RENALES que se presentaron en los pacientes VIH positivo dentro del periodo de estudio y tenemos con una mayor incidencia la insuficiencia renal aguda con 64 casos equivalente al 60%, seguido de la insuficiencia renal crónica con 21 casos equivalente al 19%, y otros 22 casos divididos de la siguiente forma, 18 casos de nefropatía asociada al VIH equivalente al 17%, y 4 casos de GLOMERULOPATIA POST-INFECCIOSA equivalente al 4% del total de pacientes en estudio. Los pacientes luego de ser diagnosticados con algún tipo de alteración renal y con el examen de control de CARGA VIRAL Y CD4, y obtuvimos que 81 casos que presentaron lesiones renales tenían de entre 50.000 y por arriba de 100.000 copias por ml de carga viral y con CD4 menor de 350 y de100 células/mm3, con 23 casos encontramos pacientes con carga viral de 10.000 a 50.000 copias/ml con CD4 de 350 a 1.000, y finalmente con 3 casos de pacientes con carga viral de menos de 10.000copias/ml y CD4 por arriba de 1.000 células/mm3. Entre los factores de riesgo con alteraciones renales encontramos en primer lugar pacientes con carga viral elevada con un 35%, seguido de pacientes con VIH más diabetes mellitus con 28%, luego encontramos los pacientes con ENFERMEDAD RENAL CRÓNICA PREEXISTENTE con 15%, pacientes con IX edad avanzada 10%, pacientes con hipertensión 8%, y finalmente los pacientes con antecedentes familiares con 4%. La frecuencia entre pacientes varones y mujeres VIH positivo con alteraciones renales y tenemos con mayor frecuencia el sexo masculino con 89 casos equivalente 83%, y el sexo femenino con 18 casos equivalente al 17%.
According to our research, we confirm the total universe of POSITIVE HIV patients registered by the Hospital, with a total of 2,346 cases, of which we disaggregate the number of patients with renal disorders, and we have 107 cases equivalent to 4% of the universe Total recorded by the hospital. RENAL ALTERATIONS presented in HIV positive patients within the study period and we have a higher incidence of acute renal failure with 64 cases equivalent to 60%, followed by chronic renal failure with 21 cases equivalent to 19%, and others 22 cases were divided as follows, 18 cases of HIV-associated nephropathy equivalent to 17%, and 4 cases of POST-INFECTIOUS GLOMERULOPATHY equivalent to 4% of the total of patients under study. The patients after being diagnosed with some type of renal alteration and with the control examination of VIRAL LOAD AND CD4, and we obtained that 81 cases that presented renal lesions had between 50,000 and above 100,000 copies per ml of viral load and with CD4 counts of less than 350 and 100 cells / mm3, with 23 cases of patients with viral load of 10,000 to 50,000 copies / ml with CD4 of 350 to 1,000, and finally with 3 cases of patients with viral load of less than 10.000 copies / ml and CD4 cell counts above 1,000 cells / mm3. Among the risk factors with renal impairment, we found first patients with high viral load with 35%, followed by patients with HIV plus diabetes mellitus with 28%, then we found the patients with PRE-EXISTING CHRONIC RENAL DISEASE with 15%, patients with age Advanced 10%, patients with hypertension 8%, and finally patients with family history with 4%. The frequency between male and female HIV positive women with renal impairment and we most often have male sex with 89 cases, equivalent to 83%, and female sex with 18 cases, equivalent to 17%.
