Resumen
La reabsorción dentinaria interna es una rara condición en dientes
permanentes que dificulta el tratamiento endodóntico. El proceso de
reabsorción dental es una condición asociada a procesos fisiológicos y
patológicos que involucra una compleja interacción entre células
inflamatorias, células que producen reabsorción y células formativas
asociadas a la matriz extracelular.
La reabsorción dental involucra tejidos mineralizados como dentina,
cemento, hueso y estructuras no mineralizadas que permiten un
intercambio biológico cuyo desequilibrio desencadena hacia condiciones
patológicas. La RI se produce por lesiones e irritaciones del ligamento
periodontal y/o de la pulpa dental y puede surgir como secuela de un
traumatismo dentario, luxación traumática, por movimientos ortodónticos,
infecciones pulpares crónicas o de las estructuras periodontales.
Altundasar y sus colaboradores la describen como resultado de una
inflamación crónica e invasión bacteriana.
Fuss y su equipo mencionan que la etiología de la reabsorción radicular
requiere de dos fases: lesión y estimulación. La lesión está relacionada
con los tejidos menos mineralizados que cubren la superficie externa de la
raíz. El tejido mineralizado denudado es colonizado por células
multinucleadas que inician el proceso de reabsorción. Sin embargo, sin
estimulación de las células odontoclásticas el proceso se detendrá
espontáneamente.
Cohen y sus colegas refieren que, histológicamente, células
multinúcleadas del tejido de granulación en la pulpa absorben la parte
interna de la raíz. La pulpa debe estar vital para que se produzca la
reabsorción.
