Estudio de sobrevida en cáncer de pulmón en el Instituto Oncológico Nacional Dr. Juan Tanca Marengo SOLCA Guayaquil 2004 - 2008

Resumen
El carcinoma de pulmón es la causa de muerte más frecuente por enfermedad neoplásica en los países occidentales, tanto en hombres como en mujeres. A pesar de todos los avances en el conocimiento de la biología del tumor, en los métodos de diagnóstico y protocolos de tratamiento, la sobrevida por cáncer de pulmón a los 5 años es de solo 1315%. En estadios avanzados tienen una expectativa de vida muy limitada, y por lo tanto las actitudes terapéuticas que se tomen pueden impactar en su calidad de vida. Por lo tanto individualizar la técnica podría ser beneficioso. El objetivo principal de este estudio es conocer la sobrevida a 5 años de los pacientes diagnosticados de cáncer de pulmón y la relación que existe con su status performance, además de demostrar el impacto sobre el pronóstico de la enfermedad. Es una investigación tipo descriptiva de diseño no experimental longitudinal retrospectivo, de una serie de casos obtenidos de los registros de la base de datos del departamento de estadística del Hospital Oncológico SOLCA Guayaquil durante el periodo 2004-2008. Los resultados fueron evaluados mediante una base de datos en sistema Excel y demostrados por curvas de sobrevida de Kaplan Meyer, gráficos de barras y pasteles con cuadros estadísticos de acuerdo a cada variable, cuyos resultados en los 192 pacientes incluidos en el estudio analizados desde la fecha en que el paciente fue diagnosticado de cáncer de pulmón hasta la fecha de ultima consulta o fallecimiento, indicando que el tiempo de sobrevida máximo en el 75% de los pacientes fue de máximo 2 meses, y en el 25% restante indica que el tiempo de sobrevida de los pacientes es de 14 meses, demostrado también por el índice de Karnofky el mismo que mejora el pronóstico mientras mas alto sea es decir mientras mejor status tenga el paciente mejor es su sobrevida. Además cabe mencionar que el estadio juega un papel importante en el pronóstico y el tiempo de sobrevida
The lung carcinoma is the most frecuently cause of death by neoplastic disease in western countries, in both men and women. Despite all the advances in knowledge of the tumor biology, in the methods of diagnosis and treatment protocols The survival of lung cancer at 5 years is only 13-15%. In advanced stages have a life expectancy very limited, and therefore the therapeutic attitudes to be taken may impact in their quality of life. Therefore identify the technique could be beneficial. The main objective of this study is to know the survival to 5 years of patients diagnosed with lung cancer and the relationship that exists with its status performance, in addition showing the impact over the prognostic of the disease. Is the research kind descriptive non experimental longitudinal retrospective design, in a series of cases obtained from the records of the database of the department of statistics of the Oncology Hospital SOLCA of Guayaquil during of period 2004-2008. The results were evaluated by a database system Excel and demonstrated by curves of survival of Kaplan Meyer, bar charts and pastels with statistical tables in accordance with each variable, the results in the 192 patients included in the study analyzed from the date on which the patient was diagnosed with lung cancer until the date of last consultation or death, indicating that the time of survival maximum in the 75% of the patient was 2 months maxim and 25% was 14 months, displayed also by the Karnofky Score, same that improve the prognostic while higher to be, that is to said while the patient with the best status, survive is best. Therefore allude that the stadium play very important role that the prognostic and the time of survive.
