Eficacia y seguridad quirúrgica de la técnica de fasciectomía palmar selectiva en pacientes con enfermedad de Dupuytren, Hospital Teodoro Maldonado Carbo, 2014 - 2015

Resumen
La enfermedad de Dupuytren, proceso de naturaleza proliferativa, de etiología aún no determinada con certeza, que afecta a la fascia palmar y da lugar a importantes deformidades en flexión con pérdida de función de los dedos de la mano e incapacidad funcional, es una patología frecuente en la consulta del Cirujano Plástico, que se ha convertido en un problema de salud pública. El objetivo del tratamiento es restaurar la función de la mano, varios métodos conservadores han sido probados la mayoría sin resultados alentadores, el manejo quirúrgico sigue siendo el tratamiento de elección. Existe aún controversia sobre la técnica correcta a ser aplicada con éxito. El desconocimiento de principios anatómico funcionales, la deficiencia en equipamiento de varias instituciones médicas y la ausencia o incorrecta aplicación de rehabilitación causan complicaciones y recidivas que resultan difíciles de solucionar en un segundo tiempo. No existen en el país trabajos sobre el tratamiento quirúrgico de esta enfermedad. Se trata de un estudio descriptivo, longitudinal, realizado en el servicio de Cirugía Plástica del Hospital Teodoro Maldonado Carbo de Guayaquil, a pacientes con diagnóstico de enfermedad de Dupuytren, intervenidos quirúrgicamente con la técnica de fasciectomía palmar selectiva, entre los años 2014 y 2015, por limitación funcional de la mano, evaluados previamente a la intervención con la clasificación de Tubiana, midiendo su eficacia funcional postquirúrgica con la utilización del método de Honner y el coeficiente de mejoría de Tubiana como métodos objetivos en consultas subsecuentes a la cirugía. Se encontró una diferencia estadísticamente significativa entre ángulos de retracción en mediciones pre y post quirúrgicas, el coeficiente de mejoría y las complicaciones fueron similares a las presentadas en la literatura mundial. La fasciectomía palmar selectiva demostró ser una técnica eficaz y segura para el tratamiento quirúrgico de la enfermedad de Dupuytren, al haber poca manipulación de estructuras anatómicas, excéresis selectiva de tejido enfermo que produce retracciones, buena visibilidad al tratarse de una técnica abierta e incisión necesaria evitando ser muy extensa.
Dupuytren's disease is a proliferative disorder of the palmar fascia and its etiology has not been certainly determined. It leads to an important flexion deformity that involves functional limitations of fingers and even functional disability. Moreover, it is a disease that a plastic surgeon frequently faces that has become a public health problem. The aim of the treatment is to restore the hand function; and even though a lot of conservative methods have been tested, the majority of them have not had encouraging results. Yet, the preferred treatment is surgical intervention. There is controversy regarding the best technique that should be applied in order to have successful results. Lack of knowledge of the principles of functional anatomy, the deficiency of the equipment in medical institutions and the absence or incorrect rehabilitation may cause difficulties and influence the chance of the recurrence. Finally, it is important to point out that in the country, there is no previous research about the surgical treatment of Dupuytren's disease. The present research is a descriptive and longitudinal study applied to patients that were diagnosed Dupuytren's disease and were surgically treated with selective fasciectomy procedure technique due to the functional limitations of their hands. Prior the surgical intervention, it was determined that patients had Tubiana’s Classification of Dupuytren’s disease. Besides, the post – surgery functional efficacy was assessed by using the Honner method and the Tubiana coefficient. These are regarded as objective methods used after the surgery. The study was performed in the Plastic Surgery Service of the Hospital Teodoro Maldonado Carbo in Guayaquil between 2014 and 2015. A statistically significant difference was found between the angles of contracture observed before and after the surgery. Furthermore, the Tubiana coefficient and the complications that were registered are similar from those stated in the literature. The selective fasciectomy procedure proved to be an effective and safe technique that can be used for the surgical treatment of Dupuytren's disease since it involves a low manipulation of anatomical structures, excision of diseased tissue that produces contractures, good visibility because it is an open technique and a necessary incision which is not too wide.
