Resumen
La preeclampsia constituye una causa importante de morbimortalidad materna, perinatal y fetal en el mundo. La Organización Mundial de la Salud (OMS) estima que existen aproximadamente más de 166 mil muertes anuales por esta patología. La preeclampsia severa es un trastorno multisistémico que se presenta después de las 20 semanas de embarazo, durante el parto y postparto, caracterizado por hipertensión arterial y proteinuria; pueden presentarse otras manifestaciones clínicas, así como alteraciones de laboratorio. Aunque el parto es el único tratamiento eficaz para la preeclampsia y a pesar que la sintomatología y las anormalidades de laboratorio suelen disminuir después de las primeras horas, el riesgo de complicaciones persiste durante el postparto. Varios reportes revisados sobre el manejo de la hipertensión arterial postparto no registran datos suficientes de alta calidad para establecer normas a seguir. Datos sobre el uso de antihipertensivos orales en el postparto son escasos; las opciones terapéuticas provienen en su mayoría de las recomendaciones de grupos de trabajo de especialistas basadas en la experiencia profesional y no en evidencias de buena calidad. El objetivo del presente trabajo de investigación será identificar qué drogas antihipertensivas orales fueron suministradas en el postparto de pacientes con preeclampsia severa para determinar el agente o esquema terapéutico que garantice un control eficaz y seguro de la presión arterial. Este proyecto de investigación será de tipo descriptivo y correlacional; y de diseño retrospectivo. Se llevará a cabo en las salas de hospitalización (cuidados intermedios I y II) del Hospital Gineco-Obstétrico Enrique C. Sotomayor. Para el efecto se revisarán las historias clínicas de pacientes con preeclampsia severa que tuvieron su postparto en las salas mencionadas. Este estudio permitirá ampliar el conocimiento sobre el empleo eficaz y seguro de la terapia antihipertensiva oral en este grupo de pacientes. Se pretende prevenir a mediano plazo las complicaciones maternas causadas por la hipertensión arterial en el postparto disminuyendo la morbimortalidad materna durante este período.
Preeclampsia is an important, perinatal and fetal worldwide cause of maternal morbidity and mortality. The World Health Organization (WHO) estimates that there are approximately more than 166 thousand annual deaths that can be linked to this disease. Severe Preeclampsia is a multisystem disorder that occurs after 20 weeks of pregnancy, childbirth and postpartum, characterized by hypertension and proteinuria; other clinical manifestations may occur, including laboratory abnormalities. Although delivery is the only effective treatment for preeclampsia and although the symptoms and laboratory abnormalities usually diminish after the first few hours, the risk of complications during the postpartum stage persists. Several reports on the management of postpartum hypertension reports do not record enough data to establish quality standards to follow. Data on the use of oral antihypertensive postpartum are scarce; therapeutic options come mostly from the recommendations of working groups of specialists based on professional experience with no good quality evidence. The objective of this research is to identify what oral antihypertensive drugs were supplied to postpartum patients with severe preeclampsia in order to determine the therapeutic agent or insurance scheme to ensure an effective and secure control of blood pressure. This research project is descriptive, correlational and observational; with a transversal and retrospective design. It will be held in the wards (intermediate care I and II) of Obstetric-Gynecologic Hospital Enrique C. Sotomayor; the sample will be conformed of stable postpartum patients hospitalized with severe preeclampsia who experienced their postpartum stage at the wards considered for this paper; this study will expand knowledge about the safe and effective use of oral antihypertensive therapy in this group of patients. It is intended to prevent (considering a midterm scope) maternal complications caused by high blood pressure thus decreasing postpartum maternal morbidity and mortality during this period.
