Diagnóstico integral del maltrato al menor, Hospital Francisco de Ycaza Bustamante 2007 - 2009, medidas preventivas

Resumen
En Guayaquil, existe un gran número de niños y adolescentes que han sufrido algún tipo de violencia, los cuales tienen como consecuencia un mayor riesgo de afecciones en el estado de salud general. El Maltrato al Menor es un fenómeno social y de salud que se ha incrementado en los últimos años, las condiciones socio-económicas críticas, que llegan a la extrema pobreza, el fenómeno migratorio, los problemas familiares, el incremento de hogares disfuncionales e incompletos, son algunas de las causas que desencadenan el maltrato, siendo los menores de edad, los más proclives a sufrirlo. El Ministerio de Salud Pública (MSP) declaró a la violencia intrafamiliar como problema de salud pública y resolvió a través de sus direcciones provinciales y ONGs mejorar la atención a los niños que sufren de maltrato como una prestación de salud financiada por el Estado. Actualmente, el diagnóstico hospitalario de Maltrato al Menor ha sido subvalorado y, a pesar de que existe en el hospital Francisco de Ycaza Bustamante, un Comité de Maltrato, éste no ofrece un tratamiento que garantice la completa integridad del menor que sufrió algún tipo de violencia. Se hace por esto, un estudio estadístico de tipo descriptivo correlacional, con un diseño no experimental, longitudinal, retrospectivo, desde enero del 2007 a diciembre del 2009 de los casos de Maltrato al Menor en el Hospital Francisco de Ycaza Bustamante. Los resultados obtenidos en el estudio mostraron una limitada cifra de diagnósticos, por escasez en su notificación, además que, tanto la negligencia como el abuso físico son los tipos de maltrato más frecuentes, en un grupo etáreo formado por menores de 5 años, siendo la mayoría de las veces originado por un familiar y que no siempre llega a ser denunciado. Toda la información fue tabulada estadísticamente y estableciendo como medida preventiva, socializar el problema con el equipo de salud del hospital para notificar a tiempo los casos, hacer seguimiento, diseñar un protocolo de acción y poder reestructurar el Comité de Maltrato.
In Guayaquil and its surrounding areas, there is a big number of children and adolescents who are victims of violence. Child maltreatment is a public health problem that has increased over the last few years. This problem has increased due to many critical social and economic factors, including poverty and extreme poverty, the high rate of dysfunctional families, the disruption of the family due to migration trends. Moreover, the youngest children are the most vulnerable. The Ecuadorian Public Health Ministry (PHM) declared family violence as a public health issue. The head of PHM instructed its provincial departments to intervene in the health care of children living in violence situations. Health care for victims of child violence has to be finance by the state, according to PHM. In the hospital “Dr. Francisco de Icaza Bustamante” it has been formed a committee called “Comite de Maltrato”, a special unit that addresses cases of violence. In spite of its existence, we hypothesize that the diagnosis of health problems related to child violence has been underestimated in the hospital. Furthermore, so far, there is no intervention that guaranties the appropriate treatment of children suffering from violence. To test that hypothesis, we are designing the present study. We will include the cases addressed by the Comite de Maltrato from January 2007 to December 2009. The results obtained in the study showed a limited number of diagnoses, reporting shortages in addition that both physical abuse and neglect are the most common types of abuse, in an age group made up less than 5 years, with Most often caused by a family member which does not always come to be reported. These results obtained were tabulated and the results statistically was established as a precautionary measure, the problem with socialized health hospital equipment to report, track, design a protocol of action to restructure the Committee on Child Abuse
