Plan educativo para mejorar la adherencia en embarazadas con vih que acuden al Hospital Mariana de Jesús

Resumen
La presencia del Virus de Inmunodeficiencia Humana (VIH) en mujeres con relación a los hombres pasó de 33% en el año 2003 a 49% en al año 2011, según datos de ONUSIDA. Esta problemática es mucho mayor en el caso de las embarazadas, pues no solo se exponen ellas sino también sus fetos ante el riesgo de infección. La falta de adherencia al tratamiento antirretroviral de gran actividad (TARGA) es la principal causa de fracaso terapéutico y por lo tanto la de mayor morbimortalidad asociada a la enfermedad. El objetivo de la presente investigación es elaborar un plan educativo para mejorar la adherencia al TARGA en embarazadas que acuden al Hospital Mariana de Jesús, mediado a través del análisis de teorías VIH, SIDA y adherencia y la caracterización de las categorías y dimensiones relacionados con la problemática de estudio, a través de una encuesta dirigida a 40 embarazadas con VIH que acudieron al Hospital Mariana de Jesús entre noviembre del 2015 y marzo del 2016. Los resultados demuestran que el 65% de las informantes no tienen suficiente conocimiento sobre la enfermedad y su tratamiento, el 38% respondió estar de acuerdo en tener buenas expectativas en su evolución post-tratamiento, el 40 % en cambio está muy de acuerdo con la capacidad del médico de aclarar sus interrogantes. Todos estos hallazgos, determinan que estas pacientes requieran una atención multidisciplinaria y un sistema coherente de acciones integrales para mejorar la adherencia al TARGA. Se concluye que existen muchos factores de tipo educativo y sociocultural relacionados a la baja adherencia al TARGA en embarazadas con VIH en el Hospital Mariana de Jesús, por lo que se propone un plan educativo para esta problemática.
The presence of the Human Immunodeficiency Virus (HIV) in women relative to men fell from 33% in 2003 to 49% in the year 2011, according to UNAIDS. This problem is much greater in the case of pregnant women, because not only them but also their fetuses exposed to the risk of infection. The lack of adherence to antiretroviral therapy (HAART) is the leading cause of treatment failure and therefore the higher morbidity and mortality associated with the disease. The aim of this research is to develop an educational plan to improve adherence to HAART in pregnant women attending the Mariana Hospital of Jesus, mediated through the analysis of HIV, AIDS and adherence theories and characterization of categories and dimensions related problems of study, through a survey of 40 pregnant women with HIV who attended the Mariana Jesus Hospital between November 2015 and March 2016. the results show that 65% of respondents do not have enough knowledge about the disease and its treatment, 38% answered agree to have good expectations in their post-treatment evolution, 40% instead strongly agree with the doctor's ability to clarify their questions. All these findings, determine that these patients require multidisciplinary care and a coherent system of comprehensive measures to improve adherence to HAART. It is concluded that there are many factors of educational and sociocultural related to poor adherence to HAART in pregnant women with HIV in the Mariana Hospital of Jesus, so an educational plan for this problem is proposed.
