Causas de ileoparalitico en niños de 4 a 10 años y su manejo quirúrgico. Hospital Francisco de Icaza Bustamante, año 2015 a 2018

Resumen
El íleo paralitico o adinámico es la afección por la cual las asas intestinales impiden el paso del contenido intestinal, lo que provoca interrupción del tránsito intestinal, alteraciones intraluminales y posterior parálisis de las asas intestinales., que puede manifestarse de diversas formas. La interrupción o detención en la continuidad normal de la progresión distal del contenido intestinal recibe el nombre de íleo. Existen dos grandes grupos: el íleo mecánico u obstructivo y el íleo paralítico o adinámico. El objetivo general será determinar las causas de complicaciones quirúrgicas de íleo en niños de cuatro a diez años, los objetivos específicos será establecer la prevalencia de íleo en el Hospital del Niño Dr. Francisco Icaza Bustamante durante el periodo 2015 a 2018, identificar las causas de íleo en los pacientes estudiados y diagnosticar las complicaciones. La metodología de este estudio es descriptiva, de enfoque cuantitativo, diseño experimental, corte transversal, retrospectivo, analítico y documental. Para obtener los resultados se escogió una muestra de 50 pacientes pediátricos con diagnóstico de íleo paralitico, donde se demostró que la mayor incidencia se presentó en pacientes de sexo femenino con 61%, la principal causa de consulta asociada a íleo quirurgo fue gastroenteritis 36%, asociada a depleción de volumen n30% y la principal complicación fue el desequilibrio hidroelectrolítico con el 54%. El 31% de los pacientes presentó íleo quirúrgico, los niños de 4 a 5 años son los más afectados con el 40% y provienen de zonas rurales, por lo que XVIII se concluye que se debería realizar un flujograma que permita el manejo rápido, oportuno y dinámico de estos pacientes que presenten signos y síntomas que demuestren que están ante un cuadro clínico de íleo quirúrgico, lo que permitirá optimizar el tiempo y manejo en los establecimientos de salud. Paralytic or adynamic ileus is the condition by which the intestinal loops prevent the passage of intestinal content, which causes interruption of intestinal transit, intraluminal alterations and subsequent paralysis of the intestinal loops, which can manifest itself in various ways. The interruption or arrest in the normal continuity of the distal progression of the intestinal content is called ileus. There are two major groups of this, mechanical ileus or also called obstructive and paralytic ileus or also called adynamic. The occlusion or intestinal blockage or obstructive ileus is a clinical picture that is characterized by the alteration in the caudal progression of the intestinal content due to an obstruction of its light caused by a mechanical cause. The obstruction can be complete, as long as the impossibility for the intestinal transit is total; or incomplete intestinal sub-occlusion when intestinal transit is difficult, but persists. The strangulation consists of the existence of a compromise of the intestinal vascularization, caused by the occlusion. XX The pseudo intestinal occlusion, paralytic or adynamic ileus consists of a compromise of the transit without a mechanical cause that justifies it, therefore, attributable to an alteration of the motor function of the intestine. The most frequent cause is previous abdominal surgery. It is usual to reserve the term of paralytic ileus exclusively for pseudo acute intestinal occlusion that affects the small intestine and the colon as a whole. The objective to determine the causes of surgical complications of ileus in children of four to ten years, establish the prevalence of ileus in the Hospital of the Child Dr. Francisco Icaza Bustamante during the period 2015 to 2018. To identify the causes of ileus in the patients studied, diagnose the complications of ileus. This study is descriptive, with a quantitative approach, experimental design, cross section, retrospective, analytical and documentary. To obtain the results, a sample of 50 pediatric patients diagnosed with paralytic ileus was chosen, where it was demonstrated that the highest incidence was in female patients with 61%, the main cause of consultation associated with ileus surgery was gastroenteritis 36%, associated with depletion of volume n30% and the main complication was the electrolyte imbalance with 54%. 31% of patients presented with surgical ileus, children aged 4 to 5 years are the most affected with 40% and come from rural areas. In conclusion, a flow should be performed that allows the rapid, timely and dynamic management of these patients who show signs and symptoms that XXI demonstrate that they are facing a clinical picture of surgical ileus, which will optimize time and management in facilities Health.
