Cavities are a global health problem that affect children; first permanent molars are the most affected dental pieces due to factors such as oral hygiene deficiency and complex occlusal anatomy. The presence of first permanent molars are of great importance because they are the key to occlusion, so premature loss would cause an occlusal imbalance. The main objective in this research is to establish the prevalence of cavities in the first permanent molars in schoolchildren from 6 to 12 years of age, in the "San Cristóbal Educational Unit" of San Cristóbal - Galapagos. A descriptive, field, cross-sectional, documentary and scientific study was carried out, in which the population studied was 361 students. Medical records were carried out, whereupon the data related to the aforementioned study were observed. The results showed that there is a prevalence of cavities in the first permanent molars of 32%; The female was the most affected one, the most affected faces was the occlusal; the age range with the highest prevalence was 10 to 12 years, the ICDAS code prevalent in cavities pieces was 2. The CPOD index was 3.7 indicating a moderate level, in the CLUNE index was shown that only 24% have the first four molars healthy, 75% have at least one surface affected or restored and 1% had lost, the IHOS index was 0.92; thus establishing a good oral hygiene level
