Eficacia y seguridad del desbridamiento enzimático en quemaduras de segundo grado profundo que afectan al miembro superior

Resumen
Introducción. Las heridas por quemaduras se caracterizan por una escara necrótica, que
cubre el lecho de la herida y se convierte en un ambiente ideal para la infección y la
cicatrización. Este tratamiento aun no es utilizado en la totalidad de pacientes afectados
dada la variabilidad en la práctica clínica ante la falta de evidencia de resultados
institucionales. Objetivo. Determinar la eficacia y seguridad del desbridamiento
enzimático en quemaduras de segundo grado profundo que afectan al miembro superior en
el hospital de especialidades “Teodoro Maldonado Carbo” en el período 2017- 2018.
Metodología. Diseño no experimental, tipo correlacional, de corte transversal en pacientes
con quemaduras de segundo grado profundo que afectan al miembro superior en el cual se
establecerá la eficacia y seguridad del tratamiento con desbridamiento. Resultados. 80
pacientes con quemadura de hombro y del miembro superior que recibieron tratamiento de
desbridamiento enzimático, de los cuales el 97.5% de los pacientes tuvieron una estancia
hospitalaria entre 1 a 20 días y disminuyeron el área de la quemadura después del uso de la
colagenasa en el desbridamiento enzimático como parte de su tratamiento de 32.5 a 18.0
centímetros en promedio; siendo estadísticamente significativo esta diferencia con p <0.05.
Conclusiones. Esta investigación contribuye a la provisión de herramientas que permitan a
los pacientes con quemaduras de segundo grado profundo que afectan al miembro superior
acceso oportuno al tratamiento con desbridamiento enzimático disminuyendo las secuelas
propias de las quemaduras.

This treatment is not yet used in all affected patients given the variability in clinical practice in the absence of evidence of institutional results. Objective. To determine the efficacy and safety of enzymatic debridement in deep second degree burns that affect the upper limb in the specialty hospital Teodoro Maldonado Carbo in the period 2017-2018. Methodology. Non-experimental design, correlational type, cross-sectional in patients with deep second degree burns that affect the upper limb in which the efficacy and safety of debridement treatment will be established. Results. 80 patients with shoulder burn and upper limb who received enzymatic debridement treatment, of which 97.5% of patients had a hospital stay between 1 and 20 days and decreased the area of the burn after the use of collagenase in the Enzymatic debridement as part of their treatment from 32.5 to 18.0 centimeters on average; this difference is statistically significant with p <0.05. Conclusions. This research contributes to the provision of tools that allow patients with deep second degree burns that affect the upper limb timely access to treatment with enzymatic debridement, reducing the consequences of burns.
