Abdomen agudo que complica el diagnóstico médico en el Hospital del Instituto Ecuatoriano de Seguridad Social Riobamba

Resumen
El Abdomen Agudo es una patología multisindrómica, siendo el motivo más común que lleva a la consulta médica; representando el 10% de las atenciones médicas en el servicio de emergencia. Su conocimiento es fundamental para dar un adecuado diagnóstico y oportuno tratamiento, que disminuya complicaciones y mortalidad por esta patología. El diagnóstico parcializado incrementa los casos erróneos y por ende una inadecuada terapéutica. El objetivo de esta investigación será determinar las causas que provocan el diagnóstico erróneo de abdomen agudo en el servicio de emergencia del Hospital del IESS-Riobamba. La metodología será un estudio cuantitativo, transversal, retrospectivo; con tabulación de datos obtenidos de las historias clínicas de los pacientes que ingresaron a emergencia con diagnóstico de abdomen agudo, entre los meses de marzo a junio del 2016. Se examinaron 197 pacientes, constituidos del 48,7% femenino y 51,3% masculino, el grupo etario con mayor incidencia es el de mayor de 70 años; los métodos diagnósticos más comunes, aplicado son: Laboratorio Clínico con el 53,2%, seguido de la Ecografía, con un 30%, En cuanto a las patologías en orden de presentación al ingreso son: Abdomen Agudo con 45,2%, Colelitiasis con el 17,8% y Apendicitis aguda con el 16,2%, frente al diagnóstico de egreso en el siguiente orden: Apendicitis aguda con el 34,5%, Colelitiasis con 23,4% y Pancreatitis aguda de 9,6%. Mostrando una diferencia significativa del 31% de diagnóstico errado; demostrando que la capacidad resolutiva con el diagnóstico clínico inicial incrementa los casos erróneos de diagnóstico definitivo. La prueba no paramétrica de Chi cuadrado con 224 grado de libertad refleja la capacidad resolutiva del diagnóstico médico inicial incrementa el diagnóstico inadecuado en la población usuaria del servicio de emergencia, aceptando el planteamiento de hipótesis nula que manifiesta la capacidad resolutiva de diagnóstico médico inicial incrementa los casos de diagnóstico inadecuado.
Acute Abdomen is a multi-syndromic pathology, being the most common reason that leads to medical consultation; Representing 10% of medical care in emergency services. Their knowledge is fundamental to give an adequate diagnosis and timely therapeutic that reduces complications and mortality from this pathology. The partial diagnosis increases the mistaken cases and therefore an inadequate therapeutic. The objective of this this investigation will be to determine the causes that the cause’s wrong diagnosis of abdomen acted in the emergency service of the Hospital of IESS Riobamba. The methodology will be a quantitative, cross sectional retrospective study. The tabulation of data obtained from the medical records of the patients who entered an emergency with a diagnosis of acute abdomen between March-June of 2016. We studied 197 patients, consisting of 48, 7% to female and 51, 5% to male; the ethnic group with the highest incidence is those older than 70 years. The most used diagnostic means are clinical laboratory with 53, 2%, followed by ultrasound with 45, 2%, cholelithiasis with 17, 8% and acute appendicitis 16, 2% compared to the order of diagnosis of acute appendicitis with the 3, 4%; cholelithiasis 23, 4% and acute pancreatitis with 9, 6% showing a significance difference of 31% of the misdiagnoses, showing that the resolution capacity with the initial clinical diagnosis increases the erroneous cases of definitive diagnosis. Statistically with the chi square test with 224 degrees of freedom shows that the resolution capacity with an initial diagnosis increases the inadequate diagnosis in the emergency service user population of the IESS Riobamba Hospital, accepting a null hypothesis that manifests that the resolving capacity of the initial medical diagnosis increases the cases of inadequate diagnosis.
