Hipertensión y diabetes como factores de riesgo de cardiopatía isquémica en Hospital Jacobo y María Elena Ratinoff

Resumen
Antecedentes: La cardiopatía isquémica continúa siendo la causa de muerte más frecuente y es la causa más común de insuficiencia cardíaca en el mundo desarrollado. Se ha demostrado que la asociación de diabetes e hipertensión arterial aumenta considerablemente el riesgo de enfermedad cardiovascular. Objetivo: Analizar la hipertensión arterial y diabetes mellitus como factores de riesgo de cardiopatía isquémica en el Hospital Jacobo y María Elena Ratinoff. Metodología: Se utilizó el enfoque cuantitativo y un diseño transversal para establecer la relación entre las comorbilidades y el riesgo de presentar cardiopatía isquémica. Resultados: Se estudiaron 100 pacientes con cardiopatía isquémica, encontrándose asociación estadísticamente significativa entre IAM, con la presencia de HTA, DM o de la asociación de HTA + DM (p <0,05). La estimación de riesgo (Odd ratio) determinó que la HTA aumenta 7 veces el riesgo de presentar IAM (OR: 7,205), que la presencia de DM aumenta 3 veces el riego de presentar IAM (OR; 3,311) y cuando existe HTA y DM aumenta el riesgo 58 veces de presentar IAM en relación al grupo que no presenta esta asociación (OR: 58,151). Se encontró asociación estadísticamente significativa entre la angina estable (AE) con la presencia de HTA, o de la asociación de HTA + DM (p < 0,05). La estimación de riesgo (Odd ratio) determinó que la HTA aumenta 2 veces el riesgo de presentar AE (OR: 2,090), que la presencia de DM duplica el riesgo de presentar AE (OR: 2,255) y cuando existe HTA y DM aumenta el riesgo 15 veces de presentar AE en relación al grupo que no presenta esta asociación (OR: 15,250) .Se encontró asociación estadísticamente significativa entre la angina inestable (AI) con la presencia de HTA, DM o de la asociación de HTA + DM (p < 0,005). La estimación de riesgo (Odd ratio) determinó que la HTA aumenta 6 veces el riesgo de presentar AI (OR: 6,119) y cuando existe HTA y DM aumenta el riesgo 22 veces de presentar AI en relación al grupo que no presenta esta asociación (OR: 22,633). Conclusiones: La edad avanzada y el sexo femenino demostraron estar íntimamente asociados al riego cardiovascular, generando mayor vulnerabilidad en este grupo de personas. La hipertensión arterial fue el factor de riesgo más importante para cardiopatía isquémica y la asociación de HTA y DM aumenta la frecuencia de eventos cardiovasculares. Existe asociación estadísticamente significativa entre el IAM, AE y AI con la presencia de HTA, DM o de la asociación de HTA + DM (p< 0,05).
Background :Ischemic heart disease continues to be the most common cause of death worldwide and is the most common cause of heart failure in the developed world. It has been shown the association of diabetes and hypertension greatly increases the risk of the cardiovascular disease. Objective: Analyze arterial hypertension and diabetes mellitus as risk factors for ischemic heart disease in Hospital Jacobo and María Elena Ratinoff. Methodology : We used the quantitative approach and the cross sectional study method to establish the relationship between comorbidities and the risk of presenting with ischemic heart disease.Results:The sample was 100 patients It wasfu Ound relation between IAM with de presence of HTA DM or HTA plus DM(p<0.005),the risk estimation rises odd ratio show that the HTA increases seven times the risk of presenting IAM ( OR 7,205). The presence of DM increase 3 times the risk of presenting IAM(OR 3,311) and when there are HTA and DM the risk increases 8 times in relation with de group that does not show that association .It was found important association between the stable angina with de presence of HTA,DM or the combination of HTA+DM (p< 0.05).the risk (Odds Ratio) show that the HTA increases 2 times the risk of presenting AE (OR 2,090) and when there is HTA and DM increase the 15 times the risk of presenting AE in relation with the group that do not present this association( OR 15,250) .the presence of DM duplicates the risk of presenting AE ( OR 2,255 ) It was found relation between the unstable angina with the of HTA ,DM or the association of HTA+DM (p<0.05).The risk estimation Odds Ratio showed that the HTA increase 6 times the risk of presenting AI (OR 6,119)and when there are HTA anDM increase the risk 22 times of presenting AI in relation with the group that does not present this association (OR 22,633) presence Conclusion : Older age and female sex proved to be closely associated with the cardiovascular risk,generating more vulnerability in this group of people .Hypertension was the most important risk factor for ischemic heart disease and association of HTA and DM increased the frecuency of cardiovascular events. There is statistically significant association of IAM ,AE, and AI with the presence of HTA,DM or of association of DM+HTA( p<0.05)
