Correlación ultrasonografía e histopatológica en el diagnóstico de cáncer de tiroides

Resumen
Antecedentes: El cáncer de tiroides (CT) es el 8vo cáncer en el Ecuador, pero con baja mortalidad. La ecografía (US) es el método más utilizado para identificar esta patología, pero su rendimiento es variable causando incertidumbre en el médico. Objetivos: Determinar la correlación entre las características ecosonográfica e histopatológica en cáncer de tiroides. Metodología: Se realizó un estudio de tipo transversal que incluyó de manera no aleatorizada 103 informes de pacientes atendidos entre el 1 de enero a 31 de diciembre de 2019 en el hospital Teodoro Maldonado Carbo de Guayaquil-Ecuador, de cualquier sexo, de 18 a 75 años indicados con US para estudio de CT. Se incluyeron pacientes sin antecedentes de CT, con estudio histopatológico de la lesión. Se excluyeron gestantes y sin diagnóstico definitivo. Para el análisis estadístico se calcularon, diagnóstico correcto (DC), sensibilidad (S), especificidad (E), valor predictivo positivos (VPP) y negativos (VPN), coeficiente de probabilidades positivos (CPP) y negativos (CPN). Resultados: Las lesiones malignas representaron el 68.9% (n= 71) frente al 31.1% (n= 32) de benignas. Con la categoría 4 en delante de TI-RADS el rendimiento mostró DC: 66.99%, S: 92.96%, E: 9.38%, VPP: 69.47%, VPN: 37.50%, CPP: 1.03, CPN: 0.75. Con la categoría 4b en adelante DC: 76.70%, S: 77.46% E: 75%, VPP: 87.30%, VPN: 60%, CPP: 3.10, CPN: 0.30. Usando la categoría 5 en adelante la DC: 43.69%, S: 19.72%, E: 96.88%, VPP: 93.33%, VPN: 35.23%, CPP: 6.31, CPN 0.83 Conclusión: La correlación del US con el resultado histopatológico del CT es moderado
Background: Thyroid cancer (TC) is the 8th cancer in Ecuador, but with low mortality. Ultrasound (US) is the most widely used method to identify this pathology, but its performance is variable, causing uncertainty in the doctor. Objectives: To determine the correlation between the ecosonographic and histopathological characteristics in thyroid cancer. Methodology: A cross-sectional study was carried out that included in a non-randomized manner 103 reports of patients treated between January 1 and December 31, 2019 at the Teodoro Maldonado Carbo hospital in Guayaquil-Ecuador, of any sex, from 18 to 75 years indicated with US for CT study. Patients with no history of CT were included, with histopathological study of the lesion. Pregnant women and without a definitive diagnosis were excluded. For the statistical analysis, correct diagnosis (DC), sensitivity (S), specificity (E), positive predictive value (PPV) and negative (NPV), coefficient of positive (CPP) and negative (CPN) probabilities were calculated. Results: The malignant lesions represented 68.9% (n = 71) compared to 31.1% (n = 32) of benign ones. With category 4 ahead of TI-RADS, the performance showed DC: 66.99%, S: 92.96%, E: 9.38%, PPV: 69.47%, NPV: 37.50%, CPP: 1.03, CPN: 0.75. With category 4b onwards DC: 76.70%, S: 77.46% E: 75%, PPV: 87.30%, NPV: 60%, CPP: 3.10, CPN: 0.30. Using category 5 onwards the DC: 43.69%, S: 19.72%, E: 96.88%, PPV: 93.33%, NPV: 35.23%, LR+: 6.31, LR- 0.83
