Resumen
Cuando se produce la pérdida prematura de un diente o dientes
temporales o hay que realizar su extracción. Los mantenedores
fijos están indicados sobre todo en la pérdida de molares y cuando
falta un solo diente, como máximo, por cuadrante. Así mismo
están indicados en pacientes en los que se sospecha poca
colaboración y en los casos que permitan la colocación de una
banda o corona en los dientes de apoyo.
Un mantenedor de espacio es un dispositivo que se coloca en los
niños principalmente, con el objetivo de impedir que los dientes se
muevan hacia delante ocupando el espacio del diente o dientes
perdidos. Estos aparatos deben: mantener el espacio durante el
tiempo suficiente hasta la erupción del diente permanente, no
interferir con el proceso de cambio dentario, impedir la extrusión
de los antagonistas, permitir la erupción del diente permanente y
restaurar la función y la estética. Se denomina fijo por que una vez
colocado no puede ser retirado por el paciente.
Existen diferentes tipos de mantenedores de espacio fijos: bandaasa,
corona-asa, arco lingual, barra transpalatina y botón de resina,
y subgingival. Cada uno de los tipos tiene sus indicaciones y
limitaciones. En todos los casos es preciso las tomas de
impresiones y la realización del mantenedor, para que
posteriormente sea ajustado y cementado en la boca del niño.
Todo niño portador de un mantenedor de espacio debe ser
controlado periódicamente para evitar desajustes y poder realizar
las adaptaciones pertinentes a los cambios fisiológicos que
ocurran hasta el momento de su retirada. Hay que vigilar y prever
el momento de su retirada.
