Valor semiológico de la creatinina en el estado de la función renal en pacientes diabéticos tipo 2 que acuden al Laboratorio Dr. José Dario Moral

Resumen
Los pacientes adultos con enfermedad renal crónica (ERC) generalmente presentan otras patologías, como diabetes o hipertensión arterial (HTA, importantes factores de riesgo de ERC. La DM es la causa más común de fracaso renal, del 20-30% de la población diabética desarrolla ERC, cuya presencia y estadio se puede diagnosticar mediante la evaluación de la tasa de filtración de glomerular (TFG), el valor referencial es en H de 94-140 ml/min/1.73m2 y en la M de 72-110 ml/min/1.73m2. En este estudio se evaluó la función renal de 50 pacientes de 42-59 y 60-80 años y, 3-20 años con DM tipo 2. Se caracterizaron mediante variables biológicas, patológicas y socio-culturales, a partir de los datos declarados en las entrevistas realizadas, después de consentir voluntariamente a participar. Además se evaluaron variables analíticas de los parámetros del estado metabólico y se incluyó la determinación de creatinina en suero y orina de 24 h para evaluar el TFG mediante la ecuación de MDRD. Para la interpretación des resultados se utilizó el paquete estadístico, software SPSS versión 21, ajustando a 95-99% de confianza (p 0,05 o p 0,01) para la asociación entre variables. Se encontró una asociación muy significante (p 0,001) entre la glucemia en dos momentos diferentes, y significativa (p-0,05) respecto a la hemoglobina glicosiladas (HbA1c), que sugirió descompensación del estado diabético de los pacientes. Este hallazgo, podría atribuirse a irregularidades con la dieta y tratamiento, posiblemente asociadas al nivel socio-cultural y, predominio de mujeres obesas dedicadas a labores domésticas. Además de una edad media de 62 9 año y tiempo medio 11,5 7,757 años con la diabetes, 92% por otro lado, el resultado muy significante (p 0,01) de los valores de depuración de creatinina (DCE) en orina de 24h, los que se correspondieron con reducción de la TFG en el 40% de los pacientes. Este hallazgo, sugiere la presencia de ERC en la muestra de pacientes, no obstante, debe confirmarse mediante la prueba de aclaramiento o DCE, por lo menos dos veces más. The interpretation of results was carried out with statistical package: software SPSS version 21, fixing 95-99% of trust (p 0,05 or p 0,01) for the association among variables. The highly significant association (p 0,001) among glycemia, in two different moments, and regarding the levels of HbA1c, indicates alteration of the diabetic state. This could be attributed to irregularities with the diet and the treatment, possibly associate the socio-cultural level and, the prevalence of obese women dedicated to domestic works. Also the 62 9 year-old half and half time 11,5 7,757 years of evolution with diabetes, 92% On the other hand, the highly significant result (p-0,01) of the values of creatinine clearance in urine of 24h belonged together with a reduction of the RGF of slight to moderate and burden in 40% of the patients. This suggests the suspicion of CKD in these; nevertheless, this should be confirmed, evaluating the FG, at least twice.
