Evaluación de los pacientes sospechosos de cáncer prostático sometidos a biopsia transrectal guíada por ecografía y su correlación histopatológica, Hospital Regional 2 del IESS Dr. Teodoro Maldonado Carbo 2009-2010

Resumen
El cáncer de próstata es una patología de gran frecuencia en el mundo. En Ecuador produce aproximadamente 720 muertes anuales. La biopsia prostática transrectal dirigida por ultrasonido es un procedimiento que se realiza en estos pacientes debido a que se encuentra incluido en el protocolo estándar para el manejo de esta patología. Sin embargo no está exenta de complicaciones por lo se ha cuestionado su uso sistemático. En el Hospital Regional 2 del IESS “Dr. Teodoro Maldonado Carbo” sus resultados no han sido evaluados. Con el objetivo de conocer los resultados obtenidos con la utilización de biopsia transrectal dirigida por ecografía en pacientes con sospecha de cáncer de próstata en esta institución, se presenta un estudio de tipo descriptivo de diseño no experimental transversal en 147 pacientes elegidos de casos sucesivos atendidos entre 2009 y 2010 en la consulta externa de urología. Para la descripción se calcularon porcentajes, frecuencias simples, promedio y desviación estándar. Los resultados indicaron que el 41% de los pacientes tenían entre 70 y 79 años. El 97% informó sintomatología urinaria (97%). El 60% tenía un grado de hipertrofia prostática III o IV. Al examen físico, la consistencia de la próstata fue dura en el 81,6%. La superficie de la próstata se informó por lo general como lisa (87,1%). Los valores séricos del APE fueron superiores a 4 en el 95,2%. Mediante biopsia se informó de Hiperplasia prostática benigna en 44% de los pacientes. Mientras que los diagnósticos de neoplasia constituyó el 43,5% de casos entre los informes de la biopsia, el diagnóstico final de neoplasias fue del 11,6%. La sensibilidad de la biopsia para detectar lesiones neoplásicas es de valor moderado (88%), mientras que la especificidad es mala al ser cercana al 50%. La información mejorará el estudio de la población de afiliados en los cuales se sospecha patología neoplásica de próstata

Transrectal prostate biopsy directed by ultrasound is a procedure performed in these patients because it is included in the standard protocol for the management of this pathology. But not without complications so has questioned its routine use. The Teodoro Maldonado Hospital, results have not been evaluated. In order to know the results obtained with the use of directed transrectal biopsy US guided, in patients with suspected prostate cancer in this institution, presents a cross-section study, in 147 patients between 2009 and 2010 in urology department. For the description percentages, simple frequency, mean and standard deviation, was used. The results indicated that 41% of patients were between 70 and 9 years. 97% reported urinary symptoms (97%). 60% had grade III or IV prostatic hypertrophy. On physical examination, the consistency of the prostate was hard at 81.6%. The surface of the prostate is usually reported as smooth (87.1%). Serum PSA values were higher than 4 in 95.2%. Biopsy was reported benign prostatic hyperplasia in 44% of the diagnoses pacientes.Mientras of malignancy was 43.5% of cases in the reports of the biopsy, the final diagnosis of malignancy was 11.6%. The sensitivity of biopsy to detect neoplastic lesions is of moderate value (88%), while specificity is bad to be close to 50%. The information will improve the population study of affiliates who are suspected of neoplastic prostate disease
