Resumen
La necrosis pulpar significa la muerte pulpar por la cual puede ser total o
parcial, en casos de caries profundas con cercanía a la cámara pulpar, el
tejido de la necrosis pulpar es la descomposición y desintegración va, de
esa manera, a permitir el libre acceso de microorganismo al conducto
radicular los cuales encontrarán ahí condiciones para multiplicación,
proliferación y propagación ocasionando la gangrena pulpar .
Se admite que ciertos microorganismos, al igual que sus productos tóxicos,
logran injuriar a la pulpa, en muchos casos, el número de bacterias que llega
a su superficie no es muy grande y si la pulpa está saludable, sus fagocitos
consiguen eliminarlas, pero cuando se produce su exposición al medio bucal,
la pulpa entra en contacto con un número muy grande de microorganismos y
es ahí donde la infección se instala de manera consistente, llevando a un
proceso inflamatorio intenso e irreversible.
Unas de las mayores dificultades en el tratamiento endodontico es
determinar hasta qué punto se debe avanzar con los instrumentos de trabajo
dentro del conducto radicular donde se debe interrumpir la instrumentación y
la obturación radicular. Lo que es inobjetable es que la instrumentación más
allá del foramen apical se debe evitarse puesto que reduciría el porcentaje de
éxito de la terapia endodontico.
Cuando ocurre una necrosis pulpar, el número de bacterias presentes
aumenta y esto debido principalmente a que los restos celulares
desintegrados sirven de nutrientes a las bacterias, además, los mecanismos
de defensas residuales y sanguíneas desaparecen del canal radicular, por lo
tanto, este hecho implica que existe un excelente ambiente para el desarrollo
de los microorganismos.
