Resumen
El conocimiento del estado de la pulpa dentaria y de los tejidos
periapicales es imprescindible para la elaboración y planteamiento de un
adecuado tratamiento. En endodoncia este procedimiento es crucial por lo
que exige todo nuestro esfuerzo y destreza, el objetivo central es
demostrar con el caso clínico, las técnicas adecuadas para realizar una
Necropulpectomía en endodoncia y aprender a realizar un correcto
diagnóstico para entregar un resultado favorable al paciente.
La patología dental, de cualquier etiología (infecciosa, traumática,
consuntiva, e incluso congénita), compromete la vitalidad del complejo
Dentino-pulpar; es necesario una terapéutica específica que responda a
las necesidades de preservar la integridad del tejido conjuntivo pulpar.
El tratamiento de conductos busca la eliminación de los microorganismos,
del sistema de conductos radiculares, los mismos que pueden
encontrarse libres en el conducto, colonizando en grado variable las
paredes del mismo o en los túbulos destinarios hasta el orificio apical.
Aunque gran parte de estos microorganismos pueden ser eliminados
durante la preparación biomecánica, ésta no alcanza todo el sistema de
conductos. En este sentido, la presencia de restos contaminados puede
hacer fracasar el tratamiento.
La necrosis pulpar generalmente es asintomática, pero hay una alteración
en el color de la dentina coronaria.
El examen radiográfico puede evidenciar restauraciones mal ajustadas o
una cavidad de caries, podemos también observar el ensanchamiento del
espacio del ligamento periodontal, denotando que jamás debemos
considerar una alteración pulpar como estanca, dado que aunque no sea
evidente, compromete al periodonto. La prueba del tallado de la cavidad
será de gran valor en los casos de dudas de diagnóstico, para estos
casos está indicada la Necropulpectomía.
