Resumen
En el desarrollo de éste tema de investigación nombraremos los temas en donde se ha vinculado al tratamiento microquirúrgico como tratamiento conservador y al tratamiento quirúrgico como un tratamiento radical, pero es necesario tener en cuenta que la cirugía nos puede permitir mantener en el medio bucal una pieza dentaria de manera parcial o definitiva según los casos.
La terapia endodóntica convencional es el tratamiento de elección para la mayoría de los pacientes con evidencia de daño pulpar y perirradicular, permitiéndonos la eliminación del proceso inflamatorio o infeccioso, conservando así el diente en función en un elevado porcentaje de casos. Sin embargo, y a pesar de las avanzadas técnicas diagnósticas y operatorias, los fracasos en éstos tratamientos aparecen en un considerable número de pacientes, estando indicada la realización de la técnica quirúrgica para poder salvar la dentición y mantener la integridad de la arcada.
Endodoncia y Cirugía caminan de la mano, quedando así obsoleta la idea que solía identificar al tratamiento ortógrado como “conservador”, y al tratamiento quirúrgico como “radical”, cuando lo que se pretende con la cirugía es la conservación de un diente que de otra forma estaría condenado a ser extraído. La Endodoncia, que en algunos países constituye una especialidad de la Estomatología conservadora. Con cierta frecuencia, asisten a consulta pacientes con inflamaciones en tejidos periapicales que presentan tratamientos pulporadiculares, los cuales han evolucionado satisfactoriamente por un tiempo. En otras ocasiones y de forma asintomática, al ser examinados radiográficamente, presentan procesos infecciosos periapicales o laterales. En la literatura se plantea que las causas más frecuentes de los fracasos de los tratamientos endodónticos son: preparación y obturación incompleta de los conductos, enfermedad

periodontal, perforaciones laterales, traumatismos persistentes y enfermedad periapical extensa. La metología que se utilizó fue científico, bibliográfica, investigativo, descriptivo. Los resultados que se obtuvieron de la investigación fueron que el porcentaje de éxito de la técnica microquirúrgica es de un 95%, se comparó con un éxito del 65% de las técnicas tradicionales. Testori y cols publicaron que el 68% de los dientes tratados con las técnicas tradicionales curaban en 4,5 años, frente al 85% con la técnica ultrasónica. El éxito clínico de la cirugía tradicional oscila entre un 44 y 90%.
In developing this research topic will name the issues where it has been linked to microsurgical treatment as conservative treatment and surgical treatment as a radical treatment , but it is necessary to consider that surgery may allow us to maintain the oral environment one piece tooth partially or permanently depending on the case . Conventional endodontic therapy is the preferred treatment for most patients with evidence of pulpal and periradicular damage , allowing the elimination of inflammatory or infectious process thereby retaining tooth depending on a high percentage of cases . However, despite the advanced diagnostic and operative techniques , failures in these treatments appear in a significant number of patients being referred to the conduct of the surgical technique to save the teeth and maintain the integrity of the arch. Endodontics and Surgery walk hand in hand , and becoming obsolete the idea that orthograde treatment used to identify as " conservative" , and surgical treatment as " radical " when what is sought to surgery is the preservation of a tooth that otherwise form would be doomed to be extracted . Endodontics , which in some countries is a conservative dental specialty . Not infrequently , patients attending consultation with inflammation in periapical tissues having pulporadiculares treatments, which have successfully evolved for a while.
