Determinación de disfunción pancreática en pacientes con esteatosis pancreática HTMC 2017 - 2018

Resumen
La esteatosis pancreática es una afección muy común, fácilmente diagnosticable, pero a menudo olvidada por médicos e investigadores. Su importancia radica en encontrar una posible correlación entre la enfermedad con la insuficiencia pancreática exocrina, ya que la evidencia actual no es exhaustiva.
Objetivo: Determinar la disfunción pancreática en pacientes con esteatosis pancreática.
Metodología: Estudio observacional, analítico, prospectivo y transversal, que incluyó a 50 pacientes con esteatosis pancreática de la consulta externa de gastroenterología del hospital regional IESS Nº 2 “Teodoro Maldonado Carbo” captados desde el periodo del 1 de agosto del 2017 hasta el 31 de julio del 2018. La gestión de datos se realizó en hojas de cálculo de Microsoft Excel 2010, donde se realizó el procesamiento de datos y para el análisis estadístico se empleó el programa SPSS 21.
Resultados: La prevalencia de esteatosis pancreática en el año 2017-2018 fue del 1% (156) de un total de 13.920 pacientes atendidos durante ese periodo. Se encontró 34% de pacientes con insuficiencia pancreática exocrina demostrada con la prueba de elastasa fecal, siendo el 94% de tipo moderada. De los 17 pacientes con insuficiencia pancreática encontrada en este estudio, el 70,60% (12) presentó infiltración grasa de tipo difusa. Se demostró asociación estadísticamente significativa entre la insuficiencia pancreática con la presencia de factores de riesgo (p = 0,01).
Conclusiones: La esteatosis pancreática se asocia a grados diversos de insuficiencia pancreática exócrina, especialmente cuando hay factores de riesgo asociados, los cuales aumentan 5 veces las probabilidades de desarrollar disfunción de la glándula.
Pancreatic steatosis is a very common condition, easily diagnosed, but often forgotten by doctors and researchers. Its importance lies in finding a possible correlation between the disease and exocrine pancreatic insufficiency, since the current evidence is not exhaustive.
