Resumen
El presente estudio aborda el tema colocación de poste de fibra de vidrio
después de una endodoncia seccionada, teniendo en cuenta que las
indicaciones más frecuentes para emplear postes prefabricados son en piezas
dentarias con pérdida del 60-70% de su remanente dentario. Existen diferentes
tipos de postes; metálicos (colados y prefabricados) y no metálicos (cerámica,
vidrio, carbono y zirconio) y su utilización dependerá de la morfología radicular,
presencia de estructura coronal remanente y fuerzas oclusales. Al momento de
realizar una cementación adhesiva, donde combinamos un cemento resinoso y
un sistema adhesivo para obtener una unión integra entre el cemento, perno y
dentina existen diferentes protocolos de acondicionamiento que se emplean
tanto en la superficie del perno como en la superficie del canal radicular,
pudiendo variar su secuencia de etapas dependiendo la marca comercial. Se
realizó un enfoque cualitativo, a través de una investigación descriptiva y
documental, utilizando el método analítico-sintético y técnico bibliográfico. Entre
los instrumentos utilizados se encuentra la historia clínica y la toma de
radiografías a un paciente, de sexo femenino de 33 años, que presenta en la
pieza #12 un tratamiento endodóntico filtrado con una destrucción coronaria que
abarca más del 60%. Se puede concluir que resulta necesario conocer el manejo
adecuado para la cementación de un poste prefabricado después de la
obturación por condensación seccional, con la finalidad de obtener un correcto
sellado entre la interfase del tratamiento endodóntico y el perno, evitando un
ajuste excesivo que pueda incitar a una posible fractura radicular.
The present study addresses the topic of fiberglass post placement after a sectioned endodontic, taking into account that the most common indications for using prefabricated poles are in tooth pieces with loss of 60-70% of their remnant Dental. There are different types of poles; (cast and prefabricated) and nonmetallic (ceramic, glass, carbon and zirconium) and its use will depend on root morphology, presence of remaining coronal structure and occlusal forces. At the time of adhesive cementation, where we combine a resinous cement and an adhesive system to obtain an integrated bond between cement, bolt and dentin there are different conditioning protocols that are used both on the surface of the bolt as on the surface of the root canal, being able to vary its sequence of stages depending on the trademark. A qualitative approach was carried out, through descriptive and documentary research, using the analytical-synthetic and technical bibliographic method. Among the instruments used is the medical history and x-ray taking of a patient, aged 33 years, presenting in the piece #12 a filteredodontic treatment with coronary destruction covering more than 60%. It can be concluded that it is necessary to know the proper handling for the cementation of a prefabricated post after sectional condensation sealing, in order to obtain a correct sealing between the endphase of the endodontic treatment and the bolt, avoiding excessive adjustment that may incite a possible root fracture.
