Resumen
Una buena relación entre la base protética y los tejidos de soporte es de gran
importancia para obtener un tratamiento rehabilitador con prótesis total removible
exitoso y longevo. Un sellado periférico deficiente, incorrecta delimitación de la
extensión de los flancos protéticos, poca retención por falta de cubertura de la prótesis
en el área de soporte acarrea dificultades durante el uso y funcionalidad de la prótesis
total removible. La presente propuesta de investigación se fundamenta en el análisis y
descripción de cada una de las consecuencias del edentulismo en la salud bucal, general
y psicológica, además de las distintas consideraciones estructurales y distintas
alternativas de tratamiento que pueden llevarse a cabo en caso de atender pacientes
edéntulos con torus palatino. También, el protocolo a seguir para la confección de la
prótesis total removible. La metodología de esta investigación se basó en la realización
de un caso clínico en la Facultad Piloto de Odontología de la Universidad de Guayaquil.
Su desarrollo se evidencio de inicio a fin a través de fotografías; conjuntamente, se llevó
a cabo la revisión y recopilación de información proveniente de varias fuentes
bibliográficas como libros, artículos de revistas y documentos de sitios web con el
propósito de recalcar la importancia de un buen diagnóstico clínico y planificación,
además de conocer mejor y a mayor profundidad la alternativa terapéutica que se ajustó
a las necesidades del caso en pro de mejorar el estado de salud del paciente otorgándole
funcionalidad y estética. Conclusiones: El éxito del tratamiento rehabilitador con
prótesis total removible se obtuvo a partir de un buen diagnóstico y del seguimiento a
cabalidad del protocolo quirúrgico y protésico puesto que la cirugía proporciono una
superficie de soporte lisa y sin interferencias permitiendo el correcto asentamiento de la
prótesis, y consecuentemente obteniendo retención y estabilidad durante la función.
A good relationship between prosthetic base and the supporting tissues is really important to get a successful and long lasting rehabilitator treatment with complete prosthesis. A deficient border molding, incorrect prosthetic flange extention delimitation, lesser retention caused by lack of coverage in the supporting area result in difficulties during the wearing and functionality of the complete prosthesis. The present research proposal is based in the analysis and description of every edentulism consequences in oral, general and psychological health, besides of the different structural considerations and distinct treatment alternatives that can be used to care edentulous patients with palatine torus, and also, the protocol to follow for the removable total prosthesis. The methodology used in this research is based in the making of a clinic case in the Pilot Faculty of Dentistry of the University of Guayaquil. Its development is evidenced from the beginning to the end through pictures, with the review and compilation of information coming from some bibliographical sources such as books, journal articles and web sites documents, in order to emphasize the importance of a clinic diagnosis and planning and also to know better and deeper the therapeutic alternative that fits to the case in order to improve the patient health condition providing functionality and aesthetics. Conclusions: the success of the rehabilitator treatment with removable total prosthesis was obtained from a good diagnosis and fully following up of surgical and prosthetic protocol and also the surgery provided a flat and no interferences supporting surface allowing the right set of the prosthesis, and consequently getting retention and stability during the function.
