Hemorragias uterinas durante el primer trimestre del embarazo. Propuesta de un protocolo de atención terapéutica

Resumen
Antecedentes. El sangrado durante el primer trimestre del embarazo es una de las
principales causas de consulta en los servicios de emergencia, que afecta a un 15-25% de
todos los embarazos. Objetivos. - Se planteó como objetivo general determinar las causas de
hemorragias uterinas en el primer trimestre de embarazo a pacientes que acuden al área de
emergencias del Hospital Materno Infantil Marianas de Jesús en el periodo comprendido de
enero a julio del 2016, para esto se consideró valorar el grado de la hemorragia mediante una
adecuada historia clínica y solicitando los exámenes de laboratorio pertinentes, confirmar la
edad gestacional por medio de ecografía y determinar si el origen de la hemorragia es
fisiológico o provocado. Metodología. - Se realizó un trabajo cuantitativo, en el periodo
comprendido entre enero a julio de 2016, la investigación fue documental, observacional no
experimental, retrospectiva de corte transversal en la que se tomaron los datos del sistema de
Registro Diario Automatizado de Consultas y Atenciones Ambulatorias (RDACAA) de un
total de 215 pacientes, y que al aplicar una fórmula de tamaño muestran fueron incluidas 100
pacientes. Resultados. - Se observó que el 87,6% de las causas de hemorragia uterina fue el
aborto (amenaza de aborto), seguido del 5,7% embarazo ectópico, y el 3,6% Enfermedad
trofoblástica gestacional y un 3,1% de masas anexiales, donde el 85% presentaron
complicaciones. Conclusión. – En el presente estudio la mayoría de los sangrados en el
primer trimestre del embarazo son causados por abortos, de los cuales el 5% fue provocado,
según muestras de fármacos encontrados, y el 95 % fueron de causas accidentales ,en su
mayoría en adolescentes la cual pone en duda si es verídico el relato de la paciente
Background. Bleeding during the first trimester of pregnancy is one of the leading causes of consultation with emergency services, affecting 15-25% of all pregnancies. Objectives. - Was raised as a general objective to determine the causes of uterine bleeding in the first trimester to patients attending the emergency area of the Marianas Maternity Hospital of Jesus in the period from January to July 2016, for this was considered to assess the extent of bleeding by applying appropriate clinical and laboratory tests relevant to confirm the gestational age by ultrasound and determine whether the source of bleeding is physiological or induced history. Methodology. - A qualitative work in the period from January to June 2016 was performed, the research was documentary, observational not experimental, retrospective cross section in which the system data Daily Record Automated Consultations and outpatient care were taken (RDACAA ) of a total of 215 patients, and applying a formula shown size they were included 100 patients. Results. - It was observed that 87.6% of the causes of uterine bleeding was abortion (threatened abortion), followed by 5.7% ectopic pregnancy, and gestational trophoblastic disease 3.6% and 3.1% mass adnexal, where 85% had complications. Conclusion. - 5% of abortions were caused, according to drug samples found, and treatment of choice in most patients was the obstetrical therapeutic curettage
