Repercusiones perioperatorias de la hipotermia no intencional inducida por anestesia general en laparotomías

Resumen
Antecedentes: La hipotermia perioperatoria es un efecto secundario común reconocido de la anestesia. Es un fenómeno frecuente pero lamentablemente no se le da la importancia necesaria y / o no se trata como se debería. Objetivos: Establecer la asociación entre repercusiones perioperatorias en pacientes sometidos a laparotomías y la hipotermia no intencional inducida por anestesia general. Metodología: Se realizó un estudio cuantitativo, observacional, analítico prospectivo en el que se incluyeron 42 pacientes consecutivos de manera no aleatoria, que fueron indicados para anestesia general por laparotomía exploratoria en el hospital Dr. Teodoro Maldonado Carbo entre el 1 de enero al 30 de marzo del 2017. En estos pacientes se tomó la temperatura por medición infrarroja, también se reportó el desarrollo de temblor, arritmia cardiaca, infarto al miocardio, pérdida sanguínea, transfusión de sangre e hipokalemia. Resultados: Las repercusiones atribuibles a la hipotermia se informaron en el 70% de los pacientes. El periodo quirúrgico en el que se presentó con mayor frecuencia fue en el transquirúrgico (62,2%). La temperatura en la que se informó la mayoría de los casos de hipotermia fue a los 35 grados centígrados (66,6%). La hipotermia no intencional se asoció significativamente al desarrollo de complicaciones. En este sentido, existió un número significativamente más alto de temblor en el grupo de pacientes con hipotermia. (11,1% vs 83,3% P < 0.001; RR 3,1081; IC 95% 1,6675 – 5,7931) Conclusión: Las repercusiones de la hipotermia en el hospital son frecuentes entre pacientes sometidas a laparotomías bajo anestesia general, siendo la de mayor repercusión el temblor.
Background: Perioperative hypothermia is a commonly recognized side effect of anesthesia. It is a frequent phenomenon but unfortunately it is not valued or it recently and / or it is not treated as it should. Objectives: To establisk the association between perioperative repercussions in patients undergoing laparotomies and unintentional hypothermia induced by general anesthesia. Methodology: A quantitative, observational, analytic and prospective study was conducted in which 42 non-random consecutive patients were included, who were indicated for General Anesthesia by exploratory laparotomy at the Teodoro Maldonado Hospital from January 1st to march 30th, 2017. In these patients, the temperature will be taken by infrared measurement, also the development of tremor, cardiac arrhythmia, myocardial infarction, blood loss, Blood transfusion, hypokalemia. Results: The repercussions attributable to hypothermia were reported in 70% of patients. The surgical period in which it was most frequently presented was in the transsurgical (62.2%). The temperature at which most cases of hypothermia were reported was at 35 degrees centigrade (66,6%). Unintentional hypothermia was significantly associated with the development of complications. In this sense, there was a significantly higher number of tremors in the group of patients with hypothermia. (11.1% vs. 83.3% P <0.001; RR 3.1081, 95% CI 1.6675 - 5.931. Conclusion: Hypothermia is a process of high frequency that has few but important repercussions during the surgeries that are performed in the hospital Teodoro Maldonado Carbo.
