Criterios para seleccionar un anestésico de uso odontológico en pacientes hipertensos

Resumen
El manejo de pacientes hipertensos se presenta como un reto para los profesionales del
área de la salud, en el ámbito odontológico están expuestos a un alto riesgo de
experimentar respuestas cardiovasculares negativas durante los tratamientos; siendo los
anestésicos locales los fármacos con mayor uso clínico en las consultas, pues permiten
realizar procedimientos y maniobras sin que el paciente experimente dolor, por ende, es
importante tener conocimiento sobre las reacciones e interacciones adversas que pueden
suscitarse como consecuencia de un mal diagnóstico asociado a dicha enfermedad
sistémica. El objetivo de este estudio fue determinar los criterios para la selección de un
anestésico de uso odontológico en pacientes hipertensos; el tipo de investigación
presentado es cualitativo y se apoya en la recopilación bibliográfica de 59 referencias
científicas, a las mismas que se les aplicó los métodos teóricos: histórico-lógico, análisis y
síntesis e inductivo-deductivo, logrando así generar la presente memoria escrita que
contiene información actualizada en referencia al tema. Además, se concluyó que para
tratar a este tipo de pacientes es necesario que su presión arterial este controlada y que no
sobrepase los 180/110 mmHg, al mismo tiempo se debe tener en cuenta la historia clínica
para conocer los fármacos antihipertensivos que le están suministrando; el anestésico local más recomendable es la mepivacaína por sus propiedades compatibles con el torrente
sanguíneo y la epinefrina como vasoconstrictor es ideal en pacientes con variabilidad de presión, representando en conjunto una combinación segura si se utiliza en dosis adecuadas.
The management of hypertensive patients is a challenge for health care professionals. In the dental specialization, they are exposed to a high risk of experiencing negative cardiovascular responses during treatments, local anesthetics are the drugs with the major clinical use in dental sessions, since they allow procedures to be performed without the patient experiencing pain; therefore, it is important to be aware of the adverse reactions and interactions that can arise as a consequence of awrong diagnosis associated with this systemic disease. The objective of this study was to determine the criteria for the selection of an anesthetic for dental use in hypertensive patients; the type of research presented is qualitative and is based on the bibliographic compilation of 59 scientific references, following theoretical methods were applied: historical-logical, analysis and synthesis and inductive-deductive, thus generating the present written report which contains updated information on the subject. In addition, it was concluded that to treat this type of patients it is necessary that their blood pressure is controlled and does not exceed 180/110 mmHg, at the same time the clinical history should be taken into account to know the antihypertensive drugs that are being administered; the most recommended local anesthetic is mepivacaine for its properties compatible with the bloodstream and epinephrine as vasoconstrictor is ideal in patients with variability of pressure,representing together a safe combination if used in correct doses.
