Utilidad de recuento plaquetario y fibrinógeno según hallazgos endoscópico como predictores de várices esofágicas en cirrosis hepática

Resumen
Varices esofágicas son dilataciones venosas o venas agrandadas en el esófago que se forma como consecuencia de la hipertensión portal y cirrosis hepática y pueden causar hemorragias graves si se rompen, la hemorragia por varices gastroesofágicas es la complicación más grave y con alta tasa de mortalidad de la cirrosis hepática y la mortalidad por sangrado de las mismas es del 15% -20%. Representa el 10% de los casos de hemorragia del tracto gastrointestinal superior. El cribado endoscópico se recomienda para la detección temprana de várices esofágicas en pacientes cirróticos con hipertensión portal. Sin embargo, este enfoque está limitado por su invasividad y costo. Objetivo: Analizar la utilidad del recuento plaquetario y fibrinógeno según hallazgos endoscópico como predictores de várices esofágicas en pacientes con cirrosis hepática. Metodología: Es un estudio retrospectivo observacional, analítico, longitudinal y de enfoque cuantitativo. La unidad de análisis será el servicio de gastroenterología del hospital de especialidades Abel Gilbert Pontón durante el periodo de 1 de enero de 2019 al 1ro de enero del 2020. La muestra estará conformada por 147 pacientes con cirrosis hepática y várices esofágicas. Resultado: Se describen las características generales de los pacientes cirróticos con várices esofágicas, se determina la sensibilidad, especificidad, valor predictivo positivo y valor predictivo negativo delrecuento plaquetario y de fibrinógeno para predecir la presencia de várices esofágicas. Se estimo el poder discriminativo de ambos biomarcadores y se establece su concordancia diagnóstica con los hallazgos de endoscopía. Conclusión: Se espera demostrar que el recuento de plaquetas y de fibrinógeno sonparámetros no invasivos con alta precisión para la predicción de várices esofágicas.
Esophageal varices are venous dilations or enlarged veins in the esophagus that form as a result of portal hypertension and liver cirrhosis and can cause serious bleeding if ruptured, bleeding from gastroesophageal varices is the most serious complication and with a high mortality rate of liver cirrhosis and the mortality from bleeding from them is 15% -twenty%. Represents 10% of upper gastrointestinal bleeding cases. Endoscopic screening is recommended for the early detection of esophageal varices in cirrhotic patients with portal hypertension. However, this approach is limited by its invasiveness and cost. Objective: To analyze the usefulness of the platelet and fibrinogen count according to endoscopic findings as predictors of esophageal varices in patients with liver cirrhosis. Methodology: It is a retrospective, observational, analytical, longitudinal study with a quantitative approach. The unit of analysis will be the gastroenterology service of the Abel Gilbert Pontón specialty hospital during the period from January 1,2019 to January 1, 2020. The sample will consist of 147 patients with liver cirrhosis and esophageal varices. Result: The general characteristics of cirrhotic patients with esophageal varices will be described, the sensitivity, specificity, positive predictive value and negative predictive value of the platelet count and fibrinogen will be determined to predict the presence of esophageal varices. The discriminative power of both biomarkers will be estimated, and their diagnostic agreement will be established with the endoscopy findings. Conclusion: It is expected to demonstrate that the platelet count and fibrinogen are non-invasive parameters with high precision for the prediction of esophageal varices.
