Resumen
Entre la diversidad de retenedores para el diseño de prótesis removible se encuentran los aditamentos extra coronales. Estos son dispositivos utilizados para la retención, fijación y estabilización de la prótesis dental; el mecanismo se encuentra ubicado fuera del contorno dentario. Este tipo de aditamento brinda dos tipos de conexiones entre la prótesis fija y la removible: rígida o elástica. Las de tipo rígida crean una restauración en extensión y las elásticas permiten cierta cantidad de movimiento el cual debe guardar relación con la depresibilidad de la mucosa que recubre el reborde residual para evitar producir torsión al diente pilar. Por lo tanto la selección de este tipo de aditamento debe estar basada en consideraciones periodontales, biomecánicas, de diseño, grado de retención mantenimiento e higiene entre otras, dirigidas a evitar daño al periodonto. Es importante para el prostodoncista conocer y aplicar este tipo de diseño para así ampliar las posibilidades de tratamiento para el paciente y resolver satisfactoriamente las demandas estéticas del mismo sin desmejorar la funcionalidad y asegurar la longevidad de la restauración y estructuras remanentes
Among the variety of retainers to design removable prosthesis are extra coronal attachments. These are devices used for retention, fixation and stabilization of the denture; the mechanism is located outside the tooth contour. This type of attachment provides two types of connections between the fixed and the removable prosthesis: rigid or elastic. The rigid type create a restore elastic extension and allow a certain amount of movement which must be related to the depresibilidad of the lining of the residual ridge to avoid producing torque the abutment tooth. Therefore the selection of this type of attachment should be based on periodontal, biomechanical considerations, design, grade retention and health maintenance among others, aimed at avoiding damage to periodontium. It is important for the prosthodontist know and apply this type of design thus extending the possibilities of treatment for the patient and satisfactorily resolve the same aesthetic demands without diminishing functionality and ensure the longevity of the restoration and remaining structures
