Resumen
El aprovechamiento de un diente muy destruido se supone tratamiento endodóntico, la reconstrucción coronaria con un muñón artificial y el recubrimiento con un corono protésica. Con ello el diente recupera la función y la estética. Cuando ya tenemos realizado el tratamiento endodóntico y el diente tratado no presenta ningún signo de patología, en ese momento podemos realizar su reconstrucción. Aquí nos planteamos la necesidad o no de colocar un poste intrarradicular. El tratamiento de un diente endodonciado termina cuando se ha restaurado y su función es completa. En la actualidad el odontólogo dispone de un arsenal terapéutico de una amplia gama de materiales y técnicas para dicho fin. Los conceptos referidos a la rehabilitación del diente endodónticamente tratado han cambiado sustancialmente. Se acepta como verdadero refuerzo a la misma estructura dentaria, por lo tanto los procedimientos que los conservan son de elección. Así mismo se deben de diferenciar las situaciones en el sector anterior y posterior de la arcada dentaria, siendo necesaria en el sector anterior una estabilidad mecánica con pernos o postes. Será en cambio. Mas habitual no utilizar dispositivos intraradiculares en el sector posterior. Se debe optar con una restauración conservadora, compatible con los resultados estéticos y funcionales. Se debe conservar lo que queda de tejido dental. Se debe reducir las tensiones y la distribución favorable de las mismas, en lo que queda de estructura dental. Esto es crucial para el éxito a largo plazo, en piezas posteriores se necesita la protección de las cúspides sin soporte para reducir la flexión y la fractura cuspídea.
