Gastric cancer and premalignant lesions constitute a high percentage of morbidity and mortality worldwide, especially in developing countries, the goal of this research is to determine the effectiveness of the method Alcian Blue in premalignant lesions of gastric biopsies of patients treated at the university hospital of Guayaquil in 2012. which will allow clinical treatment and / or appropriate surgical diagnosed by type of injury, this is one of the most recognized amplicamente histochemical techniques when you want to distinguish sulfated acidic mucin mucin tissue color staining royal blue and pale blue unsulfated. Based on the information gathered, the study found that of the 300 patients with a presumptive diagnosis of premalignant gastric lesions 38% were women and 62% were males dominate a higher incidence in males. Results: We obtained 105 samples confirmed with complete and incomplete intestinal metaplasia in 28 patients with a percentage of 27% with complete intestinal metaplasia diagnosis of 14 patients with a percentage of 13% with incomplete intestinal metaplasia and 63 patients with a percentage of 60% . Studies pre-malignant lesions of gastric metaplasia are more frequent in patients between 30-80 years of age, who have a higher prevalence among patients aged 40-49 years with a percentage of 30%. In conclusion of the 300, 105 were diagnosed with metaplasia, 118 had no premalignant gastric lesions and in 77 positive cases for the diagnosis of gastric cancer
